Hospital Regional opera bebê ainda no útero da mãe, procedimento inédito na unidade – Agência de Noticias do Governo de Mato Grosso do Sul
O HRMS (Hospital Regional de Mato Grosso do Sul) realizou, no último sábado (3), sua primeira cirurgia fetal para a correção de uma meningomielocele – malformação em um feto de 27 semanas. Até então, o procedimento só tinha sido realizado duas vezes em território sul-mato-grossense.
A malformação consiste em uma abertura na parte da coluna vertebral do bebê, deixando a raiz nervosa exposta e podendo acarretar em sérios problemas como a paralisia e hidrocefalia. O especialista em medicina fetal, Maurício Saito, que veio de São Paulo para realizar a cirurgia, explica que a incidência da meningomielocele é de um para mil nascimentos no Brasil.
“A [malformação] mais comum é a que ocorre na parte final da coluna que é responsável pela movimentação dos membros inferiores, da urina e pelo controle intestinal. Então, como há essa exposição dos nervos é muito comum a criança ficar paraplégica, não ter o controle do urina e da evacuação”, afirmou.
Para corrigir a malformação, foi utilizada uma técnica minimamente invasiva com abordagem intra-utero por endoscopia, sem a necessidade de abrir a barriga da mãe como em cirurgias convencionais, conforme detalha o especialista em cirurgias minimamente invasivas, Dr Alexandre Silva e Silva, que também veio de São Paulo para realizar o procedimento.
“Durante o procedimento a parede abdominal e o útero não são abertos. São feitas pequenas incisões de aproximadamente meio centímetro por onde passam alguns tubinhos que chamamos de trocateres. Por ele introduzimos uma câmera e os instrumentos para realizar a correção da malformação”, acrescentou o especialista.
A paciente em questão foi encaminhada para o pré-natal de alto risco do HRMS e os dois especialistas vieram de forma voluntária para executar o procedimento. “Como temos esse contato com o professor [dr Maurício Saito], ele se dispôs a vir fazer a cirurgia aqui no hospital”, completou a médica ginecologista, Vanessa Chaves de Miranda.

Residente do Serviço de Ginecologia e Obstetrícia do HRMS, Gidelson Júnior Zagonel da Silva conta como foi acompanhar um procedimento do tipo pela primeira vez.
“Encerro meu ciclo de residência em março e é a primeira vez que acompanho um procedimento dessa complexidade. Se trata de uma doença rara e quando a gente tem a oportunidade de ver um profissional qualificado e experiente, que já realiza cirurgias em São Paulo com uma frequência muito maior, isso é muito valioso”, afirmou o residente sobre sua experiência.

O paciente que acaba não realizando esse procedimento dentro do período fetal invarialvelmente será submetido, no período pós-natal, à derivação do ventrículo peritoneal – que é quando a gente coloca uma válvula para corrigir a hidrocefalia, que acaba sendo uma consequência dessa doença”, afirmou.
